ANATOMY AND BLOOD SUPPLY OF APPENDIX

ANATOMY AND BLOOD SUPPLY OF APPENDIX

Appendix where it is located it is located in the right iliac fossa which it is originates from postermedial side of the cecum about 1.7 cm from the end of the ilium the base of the appendix is located at the union of the tenia for all practical purposes the anterior tenia will end at the appendiceal origin which it is very important to detected the site of the appendix because in some cases we find difficulty to see it so we follow this tenia coli to find it   mesentery the mesentery of the appendix is derived from the posterior side of the mesentery of the terminal ileum it attaches to the cecum as well as to the proximal appendix and contains the appendcular artery which is very important and must be ligated when doing appendicetomy otherwise postappendictomy haemorrhage will occur appendicitis means inflammation of the appendix which most common causes are feacolith mean stool impact at the lumen of the appendix causing it is obstruction and this type of inflammation consider dangerous one because the appendix become liable for rupture rapid more than any type and need urgent operation other causes by lymphatic from the
wall of the appendix since the appendix containing lymphatic tissue there are another causes of appendicitis which are less commen sometimes we heard about chronic appendicitis which can come in ruptured attack and this mainly do to the presence of band making kinking for appendix and this type rare to rupture and can give rise to what means by appendicular dysphagia which simulated irritable bowel syndrome in it is symptomes and it many time misleading by physician and treat patient as (IBS)for many years and don not cure untill remove it is appendix commonest age for appendicitis from 10-30 years and there is no age excluded because we hear wrong or mistake some saying that appendicitis don not occur after age of fourty and that fatal mistake and wrong idea because appendicitis can occur at old age which it is very dangerous more than young age that is because old age associated with atherosclerosis which make appendix more liable for ruptured also we fear from appendix in children because they have short mesentery and if perforation happened there is no localization for infection so must do rapid in children also in pregnant it is fatal because uterus raise omentum and can ruptured rapid what types of appendicitis this depend on the position of the appendix presented like   1- retrocecal- retrocolic most commen 2- pelvic 3- subcecal passing downward and to the right 4-ileocecal posterior to the ileum 5- subhepatic which give symptoms like cholcystitis blood supply the appendicular artey arise from the ileocolic artery from ileal branch of the ileocolic artery or from cecal artery the artery usually single but may be double the base of the appendix may be supplied by a small branch of the anterior or posterior cecal artery the appendiceal vein lies with the artery in the mesentery of the appendix  it joins the cecal veins to become the ileocolic vein a
tributary of the right colic vein
